Traffic not routed through proxy

I am having problems with the paid proxy service using Firefox/ Foxyproxy. I have configured it according to the instructions and it worked the first time I tried it. After a reboot, I do not get a login prompt which means I am not logged into, and cannot use, the proxy. I have tried disabling the firewall on my pc, but to no effect. I am able to telnet to the proxy server: port, which shows that the connection is working from my side.
Logging shows all the URLs are going through the configured FoxyProxy proxy though they are not transmitted via the proxy server.
Any suggestions?

Make sure the proxy server

Make sure the proxy server is selected in FoxyProxy. I mean you do not have "Completely disable FoxyProxy" option selected.
Also make sure your patterns (if configured) are working properly. Possibly ther are just excluding all URLs from being proxified.
