The way to determine if your proxy requires authentication is to leave username/password fields in FoxyProxy Plus empty. Then, when you use the web through the proxy, if you get a prompt like this, you know your proxy requires authentication.
As to whether or not to check the SOCKS checkbox, you need to know if the proxy server is a SOCKS proxy or HTTP proxy. I recommend you ask your network administrator. Another way to figure it out is check IE, Firefox, or whatever browser you normally use (before you had FoxyProxy), and look at its proxy configuration settings. All browsers have a checkbox like this in their proxy settings.
